What Priornaught does

Priornaught is the pocket workbench for the US patient (and the caregiver) navigating a denied or pending prior authorization, and who knows that 82% of patient appeals succeed but only if you actually file one.

Prior authorization is the single biggest health-care burden Americans report (KFF 2026). Denial rates between plans vary tenfold. The appeal window is 60-180 days — and dies the day you stop tracking it. There is no calm app for the patient on the other side of that paper trail.

Priornaught is that workbench. No account. No insurance-portal login. No analytics. Free.

What ships in v1:

— Auth register: every prior auth in flight — service, CPT code, requested by (provider), insurance plan, request date, decision deadline, status (pending, approved, denied, peer-to-peer requested, appealed). — Plan vault: your insurance plan(s) — carrier, plan name, member ID, group, formulary tier, plan type (HMO/PPO/Medicare Advantage/Medicaid), appeal address, fax line, phone, customer-service ID. Multi-plan from day one for split households. — Denial taxonomy: bundled 30+ common denial reasons (not medically necessary, step therapy required, formulary exception, off-label, cosmetic exclusion, experimental, lifetime cap) and a one-paragraph counter-argument template for each. — Appeal timeline: an internal-appeal counter (60-180 days) and an external-review counter (60-120 days) per state, tied to each denial. Notifications fire 14, 7, 3, and 1 day before each window closes. — Communication log: every phone call, every letter, every fax, every patient-portal message — timestamped, with the name and badge number of whoever was on the other end. — Document vault: insurance card, denial letter, peer-to-peer notes, lab results, prior chart notes — Vision OCR (on-device) extracts text from each upload. — Appeal packet PDF: assembled one-tap PDF with the denial, the medical necessity letter outline, the supporting documents, and a cover sheet. — No 'Pro,' no in-app purchases, no insurance-company integration. Yours.
